bug? - videos and playlist disappeared

some videos and playlists I've loaded on KMC disappear for somedays and appear again somedays later from "entries"list but in the web site, on kaltura's player, these videos are always visible. why?

Several hours later, the file magically re-appeared. :S I deleted it as, in effect, there was now a duplicated file which was messing up my rules based playlist.
Would be nice to know what happened - and how to avoid this happening again.
Exactly the same issue here - uploaded a .MOV asset via manual upload. It completed, showed momentarily in content, allow me to generate embed code, but now it's disappeared. Except that the category it belongs to is showing one extra (as if it were there). The player I generated using this clip still plays the clip.
I re-uploaded the same file over FTP using CSV import and the second attempt worked fine and the file remains visible. Very strange!
Any idea why this is happening - and how I can delete the first, unused file?
